Do not use utl_file.fclose_all in concurrent requests…

If you expect to use output and/or log files… utl_file.fclose_all will generate an error: ORACLE error 20100 in FDPSTP Cause: FDPSTP failed due to ORA-20100: Error: FND_FILE failure. Unable to create file, l0478628.tmp in the directory… Use utl_file.fclose(p_file) instead.
